CAPs Standard:
Traditional fee-for-service model of care. We bill patient’s insurance and patient pays their copay and coinsurance based on their insurance benefits. We accept BCBS, UHC, Aetna & BCBS Anthem of Missouri.
CAPs Constant:
Concierge model of care. With $129/ month, patients receive monthly medication management visits, monthly meetings with their care coach, unlimited support via secure text message service, continuous education and support to child & their family via age appropriate written and audio-visual material. You will pay out of pocket. Your monthly payment is FSA eligible.
Program Launch date is April 1, 2020. There are limited spots which will be filled on first come first serve basis. The eligibility to the program is determined only after completion of New patient evaluation which will cost $299 (Currently discounted at $199 due to COVID-19). The duration of program is one year at a cost of $129/month. If after one year it is mutually agreed that patient still needs these services, program can be renewed again for 1 year.
Eligibility Criteria: (1) Child lives with his/her guardian (2) Child is not under the custody of State
Covered Services: (1) monthly medication management visits (2) monthly meetings with their care coach (3) unlimited support via secure text message service (4) continuous education and support to child & their family via age appropriate written and audio-visual material (5) Coordination of care with primary care provider, school and. therapist.
Services not covered under this program: (1) Any specialized testings including but not limited to psychological/neuropsychological testing for Autism Spectrum Disorder, IQ testing (2) Any legal services (3) Meetings or phone calls with someone other than primary care provider, school and therapist.
CAPs On-Demand:
Pay-as-you-go model. Fee structure: New patient evaluation: $299 (Currently discounted at $199 due to COVID-19), Medication management follow-up 25 minutes: $150, Medication management follow-up 15 minutes: $100, Medication Management with Psychotherapy follow-up 56 minutes: $300. You will pay out of pocket. We will be considered as out-of-network provider for your insurance company. We will provide you with a detailed itemized receipt (also known as super-bill) which you will submit to your insurance company. Your insurance company will reimburse you depending on your out-of-network benefits.
